Q

I was diagnosed with a "missed abortion" at 13 weeks into my first pregnancy. The ultrasound showed no heartbeat -- the embryo was approximately a seven-week size and the sac nine weeks. My nurse-midwife recommended that we wait to see if I miscarry naturally. After waiting a week and half I decided I need to get this behind me and move on. One alternative to a D&C is using the drug Misoprostol (at home) to induce the miscarriage. Your thoughts?

A

I'm so sorry that your pregnancy ended.

Misoprostol is a synthetic progstaglandin-like drug that has been used alone, as well as in conjunction with Mifepristone, for completing a miscarriage.

It does seem to be effective at emptying the uterus. Some women, however, still need a D&C or vacuum curretage.

Misoprostol can be given either by mouth (400 mcg) or vaginally (800 mcg.) Vaginal misoprostol was found to be more effective than oral misoprostol for uterine evacuation of early pregnancy failure in several studies and seemed to an effective alternative to D&C.

I think that it is definitely worth a try, because it reduces the need for surgery. The tissue would be little more than clots at this point. Most women are able to handle this better than the surgical intervention. Hopefully, you would be at home with the support of your family.

Side effects include cramping, diarrhea and nausea, but these are generally short in duration and tolerable.

I'm sorry that this happened to you and I do wish you a happy pregnancy in the near future
